Transportation

We offer door to door, non-emergent transportation for medical, dental and mental health appointments in the Mat-Su valley and Anchorage areas. Advanced notice of one week is preferred to insure availability. Anchorage transports are Tuesdays and Wednesdays only.

Outreach

Some indirect health-related transportation is available to help client’s get to the food bank, pharmacies, social service agencies and other necessities for healthy living. In direct health transportation is a secondary service of our agency and is based on availability and limited to elders or clients with disabilities.

Behavioral Health Aid Services

We offer behavioral health referral services to village residents Alaska Natives and American Indians living in our service area. Our village-based Behavioral Health Aide (BHA) makes case by case referrals for preventive, early intervention, case management, aftercare and follow-up services to individuals and families impacted by a variety of behavioral health issues that may include addiction/substance abuse and mental health problems.

Appointment Scheduling

Assistance is available for scheduling medical, dental or mental health appointments at the Alaska Native Medical Center, Southcentral Foundation’s Primary Care Center and the new Valley Native Primary Care Center in Wasilla. This service is provided to help clients reduce the long-distance telephone costs associated with these calls and to facilitate efficient delivery of health care services.

Elder Home Health Services & Chore Program

In home assistance is available to elders, disabled children & adults, and expectant mothers with special needs. Services may include; care planning, assessment of basic vital signs (monitoring blood pressure, pulse and temperature), care of non-sterile dressings, resource and referral information, health care advocacy, and light housekeeping, meal preparation and chore work.

Care Coordination

Help is available to locate agencies and services to assist clients in fulfilling their unmet needs. These include referrals to obtain health education, provider information, public assistance, Medicaid assistance, mental health services, substance abuse counseling, shelters and food banks. Coordination of care within CVH&SS is provided automatically to ensure clients receive cohesive services.

Health Education

This program coordinates community health fairs, nutrition & general health education, CPR, first aid, and more. We also provide health information literature upon request.

Social Services

The Social Services department of CVH&SS is committed to assisting clients in the enhancement of their quality of life. Services provided include the administration of BIA assistance programs, which may include time limited cash assistance for housing, food and utilities. All applicants for BIA programs must provide documentation of their Alaska Native or Native American heritage and apply with the State of Alaska’s Department of Public Assistance prior to benefits being issued. Other services provided by CVSS include resource referral for mental health services, substance abuse treatment, household management, childcare assistance, and employment needs.

~ Current GA Clients ~

If you are a current social services client wondering about your GA check and you have turned in your mandatory monthly report for the current month, please do not call and ask if we have mailed your check. When the checks are ready our case manager will call you to make arrangements. Frequent calls checking on your check only delay the process.
